How much does a vitamin D test cost in South Africa?
The test measures 25-hydroxyvitamin D, 25(OH)D, in blood. We searched for published price lists from South African pathology laboratories and pharmacy clinics and found none we could verify, so the price is not published in our sources. Prices differ by lab, province and funding route. Do not rely on figures quoted in forums or on overseas sites.
Practical steps to get a real number:
- Phone or message the lab or clinic and ask for the cash price of a 25-hydroxy vitamin D test, including any consultation or collection fee.
- Ask your medical scheme whether the test is covered and under what conditions; rules differ by scheme and plan.
- Compare a pharmacy clinic with a pathology lab, and ask whether results are sent to your doctor.
Do you need the test at all?
For most healthy adults, no. The Endocrine Society's 2024 guideline does not recommend routine 25(OH)D testing in healthy adults, and advises those under 75 to take the recommended daily intake rather than higher doses. Testing is more useful when:
- you have a bone condition, a fracture history or suspected osteomalacia
- you have kidney or liver disease, or an absorption problem such as coeliac disease
- you take medicines that affect vitamin D
- you are in a high-risk group and want to decide on a dose, for example indoor workers, people who cover most skin, older adults or people with darker skin (do South Africans need vitamin D in summer?)
How are results interpreted?
The NIH uses these reference points for serum 25(OH)D:
| Result | Meaning |
|---|
| below 30 nmol/L | associated with deficiency |
| 30 to under 50 nmol/L | generally inadequate |
| 50 nmol/L or more | generally adequate |
| above 125 nmol/L | possible harm |
If the lab reports in ng/mL, divide nmol/L by 2.5. Treatment doses for diagnosed deficiency are higher than everyday doses and should be set by a doctor.
What does supplementing cost, as a comparison?
Because we cannot quote the test, here is the other side of the decision. As of October 2026, a daily 1 000 IU vitamin D3 costs R2.16 to R3.98 (Faithful to Nature) or R2.67 (Dis-Chem Gold at Dis-Chem Living Fit). Over a year that is roughly R790 to R1 450, or about R975 for Dis-Chem Gold. Compare that with your lab quote and your risk profile. If you are low-risk, a modest daily dose with no test may be enough; if you are in a risk group, a test can prevent either under- or over-treating. The EFSA upper limit for adults is 4 000 IU a day, so stay below it unless a doctor says otherwise.
